In order to complete the online registration process, you must print and fill out the DYCD common enrollment forms and bring them in to our office. If you do not have access to a printer, you may also fill them out in person in our after school office located in room 333F or Liberty Avenue Middle School's main office.
All forms must be comp[ete before enrollment. A staff member will notify families of acceptance into the program. Acceptance into our program is on a rolling basis.
Summer Camp Registration
Pre-Registration is in Mid-March
March 21st - 3:00 - 5:00
March 22nd: 4:30 - 5:45
March 23rd: 4:30 - 5:45
Pre-Registration forms will be accepted beginning March 21st.
Summer camp forms will need to be completed once funding has been secured for summer camp.
